1. About Taché Pharmacy

Tache Pharmacy is a community pharmacy licensed by the College of Pharmacists of Manitoba (CPhM) and operating under The Pharmaceutical Act of Manitoba. We are located at 400 Tache Ave, Winnipeg, MB R2H 2A4.

All pharmacy services described on this website are provided by or under the supervision of pharmacists licensed by the College of Pharmacists of Manitoba, in accordance with the Standards of Practice established under The Pharmaceutical Act.

2. Purpose of This Website

This website provides general information about our pharmacy services, health education resources, and convenient online tools such as prescription refill requests and patient portal access.

This website does not provide medical advice. The health information presented is for general educational purposes only and is not a substitute for the advice of a physician, pharmacist, or other qualified health care provider. Always consult a health care provider for diagnosis, treatment, and answers to your personal medical questions.

Nothing on this website is intended to create a pharmacist-patient relationship. A pharmacist-patient relationship is established only when a pharmacist provides direct pharmacy services to you.

3. Online Prescription Refill Requests

Our online refill request form is a communication tool only. Submitting a refill request through this website does not guarantee that your prescription will be filled. Every refill request is reviewed and assessed by a licensed pharmacist before dispensing, in accordance with Manitoba pharmacy regulations.

When you submit a refill request, you acknowledge that:

• A pharmacist will verify the prescription, check for interactions, and assess clinical appropriateness before dispensing any medication
• You may be contacted by a pharmacist to discuss your request, provide counselling, or clarify details
• The pharmacy may decline to fill a request for clinical, legal, or regulatory reasons
• Controlled substances are subject to additional verification requirements
• Patient counselling is provided with each dispensed medication, as required by law in all Manitoba pharmacies

4. Patient Portal

If you create a patient portal account, you are responsible for maintaining the confidentiality of your login credentials and for all activity that occurs under your account. You agree to notify us immediately if you become aware of any unauthorized use of your account.

Patient portal features — including viewing prescriptions, requesting refills, and managing your profile — are provided for your convenience. They do not replace direct communication with your pharmacist. All clinical decisions remain with your pharmacy care team.

5. Personal Health Information

When you use our online services (refill requests, patient portal, or contact forms), you may provide personal health information as defined by Manitoba's Personal Health Information Act (PHIA). As a trustee under PHIA, Taché Pharmacy:

• Collects personal health information only for the purpose of providing pharmacy services to you
• Maintains administrative, technical, and physical safeguards proportionate to the sensitivity of the information
• Transmits personal health information using encryption (TLS/HTTPS) to prevent interception
• Restricts access to your information to authorized pharmacy staff who need it to provide your care
• Will notify you and the Manitoba Ombudsman in the event of a privacy breach that creates a real risk of significant harm, as required under PHIA

6. Drug Information and Health Content

Drug shortage information, generic substitution data, and health condition content on this website are provided for informational purposes. While we make reasonable efforts to keep this information current and accurate, we do not guarantee its completeness or timeliness.

Drug availability and recommendations may change without notice. Always confirm medication availability and alternatives with a pharmacist before making decisions about your therapy.

All prescription medications referenced on this website are available by prescription only and require a valid prescription from a licensed practitioner.

7. Compounding Services

Taché Pharmacy provides compounding services under the appropriate licence components issued by the College of Pharmacists of Manitoba, and in accordance with NAPRA Model Standards for Pharmacy Compounding.

All compounded preparations require a valid prescription from a licensed practitioner. Information about compounding formulations on this website is for educational purposes and does not constitute a recommendation for any specific treatment.

Compounding is performed to meet patient-specific needs that cannot be met by commercially available products. Results may vary. Discuss the risks, benefits, and alternatives of compounded medications with your prescriber and pharmacist.

8. Accuracy of Information You Provide

When using our online services, you agree to provide accurate, complete, and current information. This is particularly important for:

• Your name, date of birth, and contact information
• Manitoba Health registration number (PHIN)
• Current medications, allergies, and health conditions
• Insurance information

Inaccurate information may delay or prevent us from providing safe and appropriate pharmacy care.
