Long-acting opioid agonist for opioid use disorder and chronic pain

Methadone is a long-acting oral opioid used in opioid agonist therapy for opioid use disorder and sometimes for chronic pain. Doses are individualized; daily witnessed dispensing is common at the start of treatment.

Witnessed dosing protects the patient from drug-drug overdose risk early in treatment. Carry-home doses are introduced after stability is established.
Methadone tolerance drops quickly after several missed days. Restarting requires lower doses or a re-induction. Always contact the prescriber and pharmacy after missed doses.
Some patients stay on methadone long-term; others taper off over years. Decisions are made with the prescriber.
