Tacrolimus

Immunosuppressant for organ transplant — narrow therapeutic index

Tacrolimus (Prograf, Advagraf) prevents transplant rejection. Levels are monitored frequently. Many drug interactions and a strong food/grapefruit interaction. Brand changes need prescriber and pharmacist coordination.

How the pharmacy helps

We dispense the immediate-release Prograf and the once-daily Advagraf. They are NOT interchangeable. The pharmacist will coordinate refills with transplant clinic levels.
